Titanic wrote the first draft. Nearly every major change to maritime safety rules since has followed the same pattern: a loss, an inquiry, and an amendment that closes the gap it found. These are the ones that matter most for survival equipment.
Death tolls are given as commonly reported; several remain disputed. Rule changes are summarized — the full amendments run to hundreds of pages.
Morro Castle
A cruise liner returning from Havana caught fire at night off the New Jersey coast. The fire spread through wood-panelled interiors and open stairwells; the crew was disorganised, the distress call went out late, and several lifeboats left nearly empty. The ship drifted ashore at Asbury Park, still burning, and became a tourist attraction.
- US Merchant Marine Act of 1936 and new Coast Guard rules: fire-retardant construction, automatic fire doors, emergency generators, and mandatory fire drills.
- Crew licensing and abandon-ship training tightened; the US pushed these into the 1948 SOLAS convention’s fire protection chapter.

Andrea Doria

Two ships equipped with radar collided in fog. The Doria took a list so severe that half her lifeboats could not be launched — they were on the high side. She stayed afloat for eleven hours, which allowed a large rescue, but the stability problem was obvious.
- The 1960 SOLAS convention tightened damage-stability standards so a ship could be expected to stay upright long enough to launch boats from both sides.
- Radar training and proper use of radar for collision avoidance became a formal requirement, leading toward the 1972 Collision Regulations.
Lakonia and Yarmouth Castle
Two fires two years apart on ageing passenger ships. Lakonia burned in the Atlantic north of Madeira; some boats jammed on their davits and others swamped. Yarmouth Castle, a 1927-built ship on the Miami–Nassau run, burned so fast that her own captain left in one of the first boats. Both ships were exempt from the newer fire rules because of their age.
- 1966 SOLAS amendments applied modern fire protection standards retroactively to existing passenger ships for the first time.
- Combustible construction materials were restricted, and lifeboat launching equipment had to work with the ship listed or trimmed.
Ocean Ranger and Marine Electric
Two North Atlantic winter losses a year apart. The drilling rig Ocean Ranger capsized in a storm off Newfoundland; her crew launched a lifeboat that was smashed against the rig. The bulk carrier Marine Electric foundered off Virginia in 40-knot winds. In both cases men reached the water alive and died of hypothermia within minutes. Only three of Marine Electric‘s 34 crew survived, and all three had been in the water less than an hour and a half.
- The US Coast Guard required immersion suits on vessels in cold waters (1984), and SOLAS’s 1983 amendments to Chapter III made immersion suits and thermal protective aids part of the international standard.
- The Marine Electric hearings exposed how old ships were passing inspection; the Coast Guard created its rescue swimmer program and overhauled inspection of ageing vessels.
- Rig and ship lifeboats were redesigned around free-fall and enclosed types that can launch in heavy weather.
Herald of Free Enterprise
A cross-Channel ro-ro ferry left port with her bow doors open. Water flooded the vehicle deck and she rolled onto her side within ninety seconds, in sight of the harbour. The inquiry found no single villain but a company, in the judge’s words, “infected with the disease of sloppiness.”
- 1988 and 1990 SOLAS amendments: bow-door indicator lights on the bridge, television monitoring of vehicle decks, and new damage-stability rules for ro-ro ships.
- The finding that management failure caused the loss led directly to the International Safety Management (ISM) Code, adopted 1993, mandatory from 1998: every shipping company must have a documented safety system with a named person ashore responsible for it.
Scandinavian Star
A fire set deliberately aboard a night ferry spread through corridors lined with laminate that gave off toxic smoke. The crew, newly hired and speaking several languages, could not coordinate an evacuation. Most victims died in their cabins.
- 1992 SOLAS amendments required sprinklers and smoke detection in accommodation on existing passenger ships, not just new ones, and low-level emergency lighting to guide passengers to exits through smoke.
- Crew must be able to communicate in a common working language for safety matters.
Estonia
The worst European peacetime shipping loss since the war. In heavy seas the ferry’s bow visor tore off, taking the ramp behind it with it, and the vehicle deck flooded. She capsized and sank in under an hour, at night, in cold water. Of roughly 989 aboard, 137 survived. Many who reached rafts died of exposure before rescue arrived.
- A special SOLAS conference in 1995 brought in stricter stability rules for ro-ro passenger ships, and the Stockholm Agreement went further for Northern European ferries.
- Requirements for bow-door strength and locking were rewritten.
- Survival craft and rescue standards were pushed: better marking, lighting, and boarding arrangements for rafts; fast rescue boats on ro-ro ferries; and improved requirements for helicopter pick-up areas.
Costa Concordia
A modern cruise ship with 4,229 aboard struck rocks close inshore while performing a sail-past. Evacuation was delayed for more than an hour; passengers had not yet done a muster drill because it was scheduled for the following day.
- SOLAS amendments in force from 2015 require passenger musters before departure, or immediately after, rather than within 24 hours.
- Additional lifejackets are required at muster stations and on deck; passage planning must be reviewed by the bridge team; and lifeboat loading and lowering was reviewed.
